Families and Communities Empowered for Success (FACES)
The Family and Communities Empowered for Success (FACES) program is funded by the Department of Children and Families (DCF), and is designed to serve the uninsured population of Miami-Dade County who are co-occuring as well as those funded by Medicaid. FACES program provides Youth and their parents with mental health and substance abuse services, and assistance in identifying goals and making choices to promote resiliency and facilitate recovery. The FACES program utilizes a family-centered approach to identify the strengths and needs of the family. This multi-diciplinary treatment approach is delivered by a team composed of a Therapist, Care Coordinator, Family Coach, and Peer Specialist trained and competenet in utilizing Recovery concepts and language. The program utilizes evidenced-based strategies such as Motivational Interviewing, intensive Wraparound services, and WRAP (Wellness Recovery Action Plan). Services take place as outreach at home, school and/or office, based on family needs and choices. Intensive outpatient therapy will prevent out of home placement. FACES is based out of the Sunset Clinic and the new Homestead satellite office.
